DMD are payment arrangements, effective for twelve months from sign-up.
Payment for treatment is due at the time of service. Membership dues must
be current to receive services. Discounts cannot be combined with other
offers or insurance. Patients must schedule and keep appointments; $45
fee for cancellations with less than 48 hours’ notice. Refunds available
within 30 days; no refunds after 30 days. Renewal fees subject to change.
Two or more missed appointments or late cancellations can result in plan
termination without refund.
